How does this even happen? The article said both rear tires came off the bus. |

I saw this happen last summer in Fayetteville, WV. A dump truck headed south on US19 in the right lane was braking for the light when his right rear tires came off, hit the guardrail, bounced across three lanes and then rolled back into the median, flaming the whole time. Brake parts were flying everywhere. |

What Wagonmaster said and then some.
We have no good details as to what exactly happened, but it sounds as though the entire wheel assemblies came off (not to be confused with tires alone somehow dismounting). If it was the loss of the wheels, it may have been detectable beforehand. An internal failure may not have been visually detectable.
Maybe this is something a proper pretrip would have caught, perhaps not, regardless I hope everyone here will continue checking their buses thoroughly before each trip. Remember that rust streaks from wheel lug nuts, the presence of shiny and bright metal, missing/scraped paint are signs to watch for when checking wheels.
Watch also for oil leaking on the inside of your rims, it could indicate a failed seal from the entire hub assembly working loose.
Leaf springs, hangers, u-bolts, watch them too or you risk the loss of the entire axle as an assembly.
If in doubt about anything, seek your mechanics for confirmation.
