I remember a few years ago we had a 97' Thomas MVP ER with a CAT 3126 or 3116 engine. One day the driver discovered oil in the coolant overflow tank. What would possibly cause this issue?

A lot of things can cause this, none of them good. Transmission intercooler, engine intercooler, cylinder head gasket, cracked head, cracked block.......etc. etc.

If it doesn't fit, FORCE it.
If it breaks, well, it needed replacing anyway.

The engine in this bus was replaced shortly after the incident. I'm guessing something along the lines of a cracked head, cracked block, etc may have caused it.
