2014 IC FE, Cummins/Allison. Original DTC was poor DEF quality with engine derate. Checked DEF with refractometer and it's ok, so changed the QLS sensor and refilled DEF. I've test drove bus at different speeds and for over 2 hours at a time. The quality derate code only throws while the regular driver is on route. Is there a secondary quality sensor in the dosing head that might be malfunctioning or maybe something else entirely?

Think there is a cummins calibration update for ecm that tries to fix this issue

OEM trained in wiring and all engine platforms for over 20 years

Unless you have the dealer level Insite youll have to take it in to someone that does.

You need Insite Pro. to do the update, if you are dealing with a lot cummins engines its defiantly worth the couple extra bucks to get it. If you can get an SCR code to come up check your engine serial number on quickserve and see if its due for the TRP 2083 scr replacement.
I can tell you on BB visions when we where getting a poor DEF quality it ended up needing the SCR trp2083. But 1 of them ended up going back and finally cummins factory guy told the the dealer to change the rear NOX sensor and finally solved the problem. If you don't have quickserve send me your eng serial I will look it up for ya.
