On my 2011 BB Vision with the 6.7 Cummins, I'm getting a code saying the crankcase pressure is high and that the ccv filter requires maintainance. Before changing it, an amber light came on after 10 minutes of driving saying least severe. After changing it a red light came on saying most severe level. Am I missing something besides the filter itself?
TL;DR: High crankcase pressure, new CCV filter didn't fix it.
Yep, the vent hose is probably clogged up. Had the freeze plug rust through in the head and antifreeze filled up the top end, and lower end lol. Good luck.

The sensor might be bad. It should read zero with engine off. Ive had a few go bad already. If the really is excess crankcase pressure then you have some broken rings and need to fix them asap before it wipes out the cylinders.
